Credentialing and Privileging
Medical Staff
Medical Staff – Those providers will include:
- MD and DO
- DPM, DDS, DMD

Allied Health Practitioners – These providers will be further divided into three sub categories: Licensed Independent Practitioners, Advanced Dependent Practitioners (and other licensed practitioners that are credentialed for ANI) and Dependent Allied Health Practitioners (Dependent AHP’s).
- Licensed Independent Practitioners (LIP’s) – Psychologists, Behavioral Health Practitioners including (Doctorate & Master level Psychologists, Master level Clinical Social Workers, Addiction Specialists, Masters level Clinical Nurse Specialists in the behavioral health setting and other behavioral health specialists who are licensed, certified or registered by the state to practice independently) and Certified Nurse Midwives
- Advanced Dependent Practitioners – Physician Assistants, Nurse Practitioners, and Certified Nurse Anesthetists. Along with other licensed practitioners for ANI credentialing only – Audiologists, Physical Therapists, Occupational Therapists, Speech-Language Pathologists.
- Dependent Allied Health Practitioner (Dependent AHP) – Surgical Tech’s and Nurse Clinicians
